Box 5022, Department 852962, Kalamazoo, MI 49003-5022. Include the Region of the prize drawing that you would like to participate in on the front of the envelope. Failure to include the name of a Region will result in forfeiture of your Sweepstakes entry. You will receive one (1) entry into the applicable Region’s Sweepstakes drawing.
