Box 5022, Department 851006, Kalamazoo, MI 49003-5022. Limit: One (1) Code request per envelope. All mail-in requests must be handwritten and must be postmarked by May 17, 2022 and received by May 24, 2022.
Online Free Code Request -- From May 18, 2022 to May 31, 2022: To request a Code online, visit www.OuiMerciBouquet.com and follow the links and instructions to complete and submit the Code request form, including a valid email address, and you will receive a Code within one (1) business day via automated email. All requests must be received by May 31, 2022 at 11:55 p.m.
