Box 5010, Department 843789, Kalamazoo, MI 49003-5010. You will receive 5 entries into the drawing for this Sweepstakes per request. Limit: 1 request per stamped envelope. All mail-in entries must be handwritten, postmarked by May 12, 2020, and received by May 19, 2020.
