(10) Grand Prizes. Each Grand Prize will consist of:

To enter without purchase, hand-print your Full Name, Complete Mailing Address, E-mail Address and Phone Number and Birth Date on a plain 3x5 inch paper. Each entry must be mailed separately in a hand-addressed, stamped envelope.
