A signed soccer ball from a Sponsor-selected athlete will be awarded to Nominee (ARV: $300 each).
A Block Party for the Nominee’s soccer team
The non-grant portion of the Grand Prize has a total ARV of $1,000.

A signed soccer ball from a Sponsor-selected athlete awarded to the Nominee (ARV: $300 each).

Must be legal residents of the 50 United States and the District of Columbia, eighteen years of age or older as of date of entry, who are the parent or legal guardian of a child seven to fifteen years of age at the time of entry who are active soccer players (e.g., recreational, school-based, or club level) and are members of a soccer team or soccer organization that is recognized as a tax-exempt nonprofit organization under the Internal Revenue Code. Nominees must be the child/charge of the Nominator and a legal resident of the 50 United States and the District of Columbia, seven to fifteen years of age as of date of entry and an active soccer player.

Limit of one Entry per Child.

Nominators may enter the Promotion by visiting CVSHomeTeamHeroes.com and completing the online entry form, including sharing the name of their child/charge who is seven to fifteen years of age (at the time of entry) and is an active soccer player to nominate and answering the following questions: “How is your child helping make a difference in the community?”; “How has soccer shaped who your child is on and off the field?”; and “What does being crowned a ‘Home Team Hero’ mean to your child and their community?” in 250 words or less per question. Then, Nominators must upload three recent photos (2 soccer participation photos and 1 community involvement photo) of their Nominee.
