Open to legal residents of the fifty United States and the District of Columbia who are Healthy Rewards Loyalty Program Members and age 18 or older at the time of entry. Membership in the Healthy Rewards Loyalty Program is free.

There is a limit of three entries per person per day, regardless of entry method.

Upon completion of the entry form, the entrant will receive one entry into the Game, and Sponsor will initiate a Spin in the system on behalf of entrant. The system will also display key information required for your mail-in submission, which is required to determine the outcome of your play. Neatly write or print out and tape the key information that was displayed upon submission of your form on a 3x5 postcard. All information displayed must be provided on the postcard to qualify. Entries must be postmarked no later than three calendar days after the end of the Entry Period.
