Monthly Prizes: There are four (4) monthly prizes available to be won each Monthly Entry Period (each, a “Monthly Prize”). Each Monthly Prize winner will receive one (1) $500 Staples Gift Card. The approximate retail value (“ARV”) of each Monthly Prize is: $500.

The total ARV of all prizes available to be won: $24,000.

Open to legal residents of the 48 contiguous United States, including the District of Columbia, who are age 18 or older at the time of entry (or the age of majority in the jurisdiction of their residence, whichever is older).

Limit: One (1) Entry per person per Monthly Entry Period regardless of the method of entry.

Method #2: AMOE (Alternate Means of Entry) - To enter the Sweepstakes without making a purchase at a Staples store or without completing the survey, hand print your name, complete address, city, state, zip code, daytime phone number, email, and birth date on a 3” x 5” card and mail it to: Staples Customer Satisfaction Survey $500 Monthly Sweepstakes Entry, PO Box 728, Social Circle, GA 30025-728. Mailed entries must be postmarked by the Entry Period End Date and received by the date indicated in the chart above, to be eligible to participate in the Drawing for that Monthly Entry Period. Each mail-in entry must be mailed in a separate outer envelope.
