VIA MAIL: On a plain piece of 3” x 5” paper, hand print your full name, street address, city, state, zip, date of birth (mm/dd/yyyy), daytime telephone number and valid email address (collectively “Mailed Entry” or “Entry”). You must also include your signature on your Entry to certify that you are 21 years of age or older. Mail your Entry in a hand-addressed envelope via first-class mail to: Modelo College Football Playoff 2024 Southern Region Sweepstakes Entries, P.O. Box 7706, Melville, NY 11775-7706, to receive one (1) Entry into the Sweepstakes. IMPORTANT: You must indicate the 2-letter postal abbreviation for your state of residence in block letters in the lower, left-hand corner of the front of your envelope.
