One (1) Grand Prize Winner: Grand Prize winner will receive a four (4)-night trip for two (2) (winner and one (1) guest) to their choice of one (1) of the following destinations A) New York, NY; B) Los Angeles, CA; C) Miami, FL; or D) Chicago, IL (hereafter, “Trip”). Trip includes round trip coach air transportation from the major airport nearest winner’s residence (determined by Sponsor) to selected destination, ground transportation to/from airport, standard double-occupancy hotel accommodations, select onsite activities, one (1) $500 Mastercard Prepaid® card (for use by winner only), and $3,000 awarded as a check to the winner to be used to help offset taxes (“Grand Prize”). Sixty (60) First Prize winners (ten (10) First Prize winners per Entry Period): Each First Prize winner will receive a $200 statement credit on their Ulta Beauty Rewards Mastercard account* (“First Prize”). (ARV of each First Prize = $200). In the event a non-cardholder is selected as a First Prize winner, prize will be awarded in the form of a check.

Enter as often as you wish by mail, but each entry must be mailed separately, postmarked by the applicable postmark deadline and received by the applicable mail-in deadline for each Monthly Period (see rules).
