Open only to individuals who are legal residents and physically located in one (1) of the following U.S. States ONLY: AZ, CA, CO, CT, FL, GA, HI, IL, KY, LA, MA, MD, MI, MO, NC, NJ, NM, NV, NY, OH, OR, PA, TN, TX, VA, or WI, who are at least eighteen (18) years old as of time/date of participation, and who either live or work within a 25-mile radius of a participating CPK restaurant.

ALTERNATE MEANS OF OBTAINING A CARD (3x5 mail-in card or paper method of request): If you are eligible per Section 1, above, during the Distribution Period, to request a Card without making a purchase or visiting a CPK restaurant, hand-print your full name, full mailing address, email address, phone number, and birth date on a plain 3x5 inch paper or card. You must include a self-addressed, stamped envelope which will be used to send you a Card via return mail. Mail your completed request to California Pizza Kitchen, Attn: Marketing Department – Thank You Promotion – Card Request, 575 Anton Blvd. #100,Costa Mesa, CA, 92626.
Regardless of how a Card is obtained, to be valid, a Card must Opened by a Restaurant Manager, during the Distribution Period, and must not be opened by the participant. Opened Cards are invalid and will not be honored.
