Step One: You must first register for the Sweepstakes online between 12:00:01 AM ET on Tuesday, September 1, 2020 and 11:59:59 PM ET on Friday, October 30, 2020. During the Sweepstakes Registration Period, register for the Sweepstakes by visiting www.allerganaestheticspromotion.com/look-forward and follow the instructions to complete and submit the sweepstakes registration form.
Step Two: Mail a postcard, including your name, complete address, email address, and phone number to the following address to receive one entry into the Sweepstakes: The Look Forward Sweepstakes, 625 Panorama Trail, Suite 2100, Rochester, NY 14625-2437. Each Mail-In Entry must be mailed separately and must be postmarked between Tuesday, September 1, 2020 and Friday, November 6, 2020 and received by Friday, November 13, 2020. Photocopied, illegible, or mechanically reproduced entries are not eligible. Sponsor is not responsible for lost, late, damaged, misdirected or postage-due mailed entries.
