Mail-in in lieu of the Receipt method of entry: To receive additional entries into the Sweepstakes without submitting a valid unique receipt code, Entrant may enter by mail as follows: On a plain 3" x 5" card, legibly handprint or type your first name, last name, complete address, city, state, zip code, home phone number (including area code) and email address (if any) and mail an envelope containing your completed 3" x 5" card to: "O'Charley's Date Night Getaway" Sweepstakes PO Box 10367, Rochester, NY 14610. Each envelope containing a properly completed 3" by 5" card shall equal one entry. Limit one card per envelope.
