(1) Prize package consisting of the following: 1) Two round-trip coach class airline tickets from any major city served by major airlines in the 48 contiguous United States to Phoenix, Arizona (If winner resides within Arizona, ground transportation shall be substituted); 2) two nights hotel stay (single room, double occupancy); 3) transportation to and from PHX airport, hotel, and Bret Michaels' home, 4) two spa packages, 5) one dinner for two with Bret Michaels, and 6) two pre-paid credit cards for $225 ($450 total). ARV of the prize package is less than $5,000.

Open to legal residents of the 48 contiguous United States and the District of Columbia (excluding Hawaii, Alaska, Puerto Rico, U.S. Virgin Islands and other commonwealths, territories and possessions) who are 21 years of age or older.
