Follow the instructions to print a physical copy of the National Pizza Return form and complete the form with your full name, mailing address, email address and respond to all of the questions provided on the form. Then, mail your completed form to the following address to receive one (1) entry; Olson Attn: Pizza Hut/Pizza Return, 564 W. Randolph, Fl. 5, Chicago, IL 60661. Entries must be postmarked no later than April 18, 2015 and received by April 21, 2015 to be considered. Your responses to the National Pizza Return form questions will not increase or decrease your chances of winning a prize.
