One (1) Grand Prize: A trip for the Grand Prize winner and one (1) guest to San Francisco, California to attend Super Bowl 50 on February 7, 2016. Grand Prize includes: Round-trip, coach-class airfare for the Grand prize winner and one (1) guest to San Francisco, California from an airport determined at the sole discretion of the Sponsor; four (4) nights’ hotel accommodations (hotel to be determined at the sole discretion of the Sponsor, in one (1) double-occupancy rooms, excluding room service, meals, and other incidentals); two (2) tickets for the Grand Prize winner and guest to attend the Super Bowl 50 (seats to be determined at the sole discretion of the Sponsor); and $500 spending cash to the Grand Prize winner only, awarded in the form of a bank gift card, intended to be used for meals and ground transportation. The Grand Prize winner will also receive a single cash payment in the form of a corporate check to the winner only. The single cash payment will be based on a 34% tax rate of the final Grand Prize value, which may vary based on factors described below. The single cash payment is intended to assist with any federal or state tax obligations related to the Grand Prize, but in no event will the single cash payment exceed $3,348.48. Any additional tax obligation that the Grand Prize winner may be liable for is the sole responsibility of the winner. The Grand Prize winner is responsible for all federal, state, and local income taxes and other taxes associated with the prize. The Sponsor will comply with all tax reporting requirements. HOW TO ENTER: There are two (2) ways to enter - either i) online using an Xbox One console, or ii) via the U.S. mail:
a) To enter online via an Xbox One console, you must be an Xbox Live Gold or free member in good standing at the time of entry/winning and have access to an Xbox One console. Once you have verified that you meet these system requirement, you must sign in to the Xbox Live service via an Xbox One console, navigate to the Apps Channel on the Xbox Live dashboard, download the free NFL Application (“App”) if you have not already done so, click the designated area with the App as instructed, verify eligibility, and accept the Official Rules and Microsoft’s Privacy Statement, and click “Submit”. You then will have the opportunity to answer trivia picks on screen. The quantity of trivia picks available during each Entry Period are as follows: during Entry Period #1 and #2, up to twenty two (22) picks; Entry Period #3, up to fourteen (14) picks; and Entry Period #4 and #5, up to ten (10) picks. For each trivia pick you answer, you will receive one (1) entry into the applicable Entry Period drawing, regardless if the answer you provide is correct or incorrect.
b) To enter via mail, hand-print your first and last name, address (no P.O. Boxes accepted), city, state, zip code, daytime phone number, and e-mail address, on a 3” x 5” piece of paper, insert it into a business envelope and mail it with sufficient postage to:

Mail-in entries must be postmarked by the end date of each Entry Period and must be received within seven (7) calendar days from the end date of an Entry Period to be eligible. Limit one (1) entry per outer mailing envelope. Neither the Sponsor nor its affiliated or related entities are responsible for lost, late, incomplete, inaccurate, misdirected, damaged, unintelligible, postage-due, or returned entries.
