Complete and submit the “VIZIO This One’s For Mom” entry form by: 1) filling out your name, e-mail address, phone number, birthdate, zip code; b) submit either a video that is no longer than two (2) minutes in duration, or a photo via the entry form with an accompanying story (not to exceed 1,000 characters in length) explaining why your Mom is the greatest; and c) electronically agree and accept these Official Rules and click the “Submit” button.
