One (1) Grand Prize: Grand Prize winner will receive: round - trip air travel to Orlando, FL for winner and three (3) guests either September 25 – September 29, 2014 or November 6 – 10, 2014; round - trip ground transportation between the Orlando International Airport and Walt Disney World ® Resort for four (4) people; four (4) nights accommodations at a Value Resort at the Walt Disney World ® Resort (in Sponsor’s sole discretion) including one quad occupancy standard room; sixteen (16) one day/one park theme park tickets to the Walt Disney World® Resort; and $100 per person per day spending money, in the form of a Visa® Gift Card ($2,000 total) for food and any expenses. Sweepstakes is only available in the 48 contiguous United States and District of Columbia (collectively the “Territory”) and is open to residents of the Territory who are 21 years of age or older at time of entry.

HOW TO ENTER: Consumers that visit one of the Visionworks participating retail locations in the Territory and see a demonstration of Transitions Adaptive Lenses can receive a pre - paid Sweepstakes Entry Postcard (“ Postcard ”). NO SALES PRESENTATION REQUIRED. To enter the Sweepstakes, the consumer (each an " Entrant ") should complete the required information on the Postcard and mail the Postcard to: Transitions Adaptive Lenses Sweepstakes at Visionworks , P.O. Box 7465, Prospect Heights, IL 60070 which must be postmarked by May 31, 2014 and be received by Sponsor no later than June 6, 2014. When the Postcard is received, the Entrant will receive one (1) entry into the Sweepstakes. ENTER WITHOUT A STORE VISIT: During the Entry Period, on a piece of 3” x 5” paper, print your complete name, residential address including city, state and zip code, daytime and evening telephone numbers, your age in years, valid email address (optional), and the name of a Visionworks retailer near your residence (“ Paper Entry ”) and mail via first class mail to: Transitions Adaptive Lenses Sweepstakes at Visionworks, P.O. Box 7465, Prospect Heights, IL 60070 postmarked by May 31, 2014, and to be received no later than June 6, 2014. Each Paper Entry must be in a separately postmarked outer envelope.
