Initial Sweepstakes Prize consists of the following: A trip for the Initial Sweepstakes Winner and his/her (1) guest to London, England to attend selected events at the 2012 London Olympics. London Trip includes: round-trip, commercial coach class air transportation to an airport near London, England; standard accommodations for (2) nights at a hotel in London; round-trip ground transportation between London Airport and London Hotel; (2) tickets to (3) separate Olympic events; and Initial Sweepstakes Winner (only) will receive $2,000 paid in the form of a check. ARV: $16,060.

Second Sweepstakes Prize: There will be (2) prizes awarded to (2) winners for the Second Sweepstakes Period. Each Second Sweepstakes Prize consists of (1) 42” HDTV. ARV of each Second Sweepstakes Prize is $600.

Third Sweepstakes Prize: (1) item of Olympic memorabilia autographed by a current or former Olympian. ARV of Third Sweepstakes Prize is $200.

Fourth Sweepstakes Prize: (1) item of Olympic memorabilia autographed by a current or former Olympian. ARV of Fourth Sweepstakes Prize is $200.

Fifth Sweepstakes Prize: (1) 42” HDTV. ARV of Fifth Sweepstakes Prize is $600.

Sixth Sweepstakes Prize: A trip for Winner and his/her (1) guest to Colorado Springs, CO to tour the United States Olympic Team training facilities. Colorado Trip includes: round-trip commercial coach-class air transportation; standard hotel accommodations at the Broadmoor Resort for (2) nights; round trip ground transportation to and from Colorado Airport and hotel; and the opportunity for Sixth Sweepstakes Winner and Sixth Sweepstakes Guest to tour the United States Olympic Team training facilities. The ARV of the Sixth Sweepstakes Prize is $3,600.

Total ARV of all Prize is $21,860.

Open only to permanent, legal United States residents physically residing in the (50) U.S. or the District of Columbia who are at least (18) years of age or older and of the age or majority in his or her state of residency as of June 15, 2012.

There are (2) ways to enter the Promotion: (1) By submitting information regarding your cable, satellite, or video service provider on the Promotion web site; and (2) by mail.

Limit (1) Entry per person/email address for the entire Promotion Period.

To enter by mail without submitting information regarding your cable, satellite, or video service provider account: If you are not a current cable, satellite or video service provider customer in good standing as of the date of your Entry, or if you wish to enter the Sweepstakes without submitting information regarding your cable, satellite, or video service provider account, then do the following:

Legibly hand-print your first and last name, complete home address (including Zip Code), daytime telephone number (including area code), date of birth, and email address (optional) on a postcard (or piece of paper no larger than 6” x 8”) and mail the Postcard (or piece of paper in a business size (#10 envelope) with first-class postage affixed) to: The NBC Olympics Live Extra Sweepstakes ENTRY, P.O.
