Trip consists of roundtrip coach air travel from major commercial airport nearest winning Nominee’s residence to Dallas, Texas (airports are of Sponsor’s sole choosing); standard double occupancy accommodations; $500 gift card from a ride-hailing service provider; two VIP tickets to Kane Brown concert; $2000 check which may be used by winning Nominee for taxes incurred by prize award.

NOMINATORS/NOMINEES MUST BE A LEGAL UNITED STATES RESIDENT OF THE 50 UNITED STATES OR THE DISTRICT OF COLUMBIA AND 21 YEARS OF AGE OR OLDER. NOMINEES MUST ALSO BE A RETIRED MEMBER OF THE U.S. MILITARY (NOT ACTIVE DUTY) WHO WAS HONORABLY DISCHARGED FROM MILITARY SERVICE.

A Nominator may only submit one nomination.

Complete the nomination form explaining why the Nominee deserves to be recognized with a prize trip to see Kane Brown in concert. Be sure to identify the Nominee by their first and last name and branch of the U.S. Military in which they served.

Each valid nomination will result in a charitable donation by Sponsor to CreatiVets, a non-profit entity organized under Section 50lc(3) of the Internal Revenue Code, with an address of 1123 12th Ave S, Unit B, Nashville, Tennessee 37203 and Packages From Home, a non-profit entity organized under Section 501c(3) of the Internal Revenue Code, with an address of 5643 N. 52nd Avenue, Glendale, Arizona 85301. Maximum donation of $50,000 ($25,000 for CreatiVets; $25,000 for Packages From Home). For clarity, neither Nominee nor Nominator will be (a) eligible for any tax deduction or similar benefit in conjunction with such donation or able to choose which non-profit entity receives a charitable donation.
