Rules
Offered only to individuals who are legal residents of the fifty (50) United States or District of Columbia, at least eighteen (18) years old. To participate in the rewards program only, you must have internet access and a valid email address at the time of participation in the Promotion.
For each Code that is entered, received and recorded by the Promotion computer you will receive the point value as stated in Rule 4 below, as well as one (1) entry into the Sweepstakes. While you can deposit as many Codes as you wish into your Account there is a limit of (100) entries into the Sweepstakes per person/Account/email address/street address or household during the Promotion Period, regardless of method of entry.
To Participate ONLY In The Sweepstakes Drawings By Mail: On a 3"x5" card hand print your name, complete address, daytime telephone number and age. Mail in a first class, stamped envelope to:

Passwords. Each time you visit the Web site to participate in the Promotion, you must log-in using your email address and password. If at any time you forget your password, you may seek to gain access to your password by clicking on the "Forgot Your Password" link on the Web site. Follow the on-screen instructions and your password will be emailed to you at the email address provided at the time of registration. You are solely responsible for the security of your password. You should take precautions to keep your password private to prevent unauthorized use. Neither Sponsor, YTBS nor any other person or entity will be responsible for any losses, damages or consequences you may suffer due to careless or unauthorized use of a password.
Only one (1) password may be used by any participant and there is a limit of one (1) Account per person/Account/email address/street address or household. Should any participant attempt to use more then one (1) password or establish more than one (1) Account, that participant may be disqualified.
HOW TO PARTICIPATE:
To participate in both the Sweepstakes and Rewards portions of the Promotion via the Internet, you must first obtain a Code or Codes through one of the methods described below, and then register the Code(s) at www.lavicrewards.com (the "Web site") by following the instructions below.
Obtaining a Code.
